Open your old OFF wallet (the one that controls the address above) and produce a signmessage proof. Run this exact command in a terminal:

Offerings-cli signmessage <address> "<challenge>"

You'll get back a base64 signature string. Paste it here:

Base64 signature * ~88 characters ending with =. Whitespace will be stripped automatically.

Signature verified. Fill in the rest of the claim:

New-chain destination address * Where the disbursed OFF will be sent. Must be a Q-prefix address you control.
